Post-ER care for chest pain reduces risk of heart attack, death

Mar. 27, 2013 ? Follow-up care after an emergency room visit for chest pain significantly reduced the risk of heart attack or death among high risk patients. Chest pain is the most common reason patients go to the ER. However, one in four chest pain patients didn't seek follow-up care within a month, as recommended.

Seeing a doctor within a month of an emergency room visit for chest pain significantly reduced the risk of heart attack or death among high risk patients, according to research published the American Heart Association journal Circulation.

Chest pain is the most common reason people go to the emergency room in developed countries and accounts for more than 5 million ER visits each year in the United States.

The study is the first to demonstrate the importance of follow-up care for chest pain patients after leaving the ER, researchers said. High risk patients in this study were those with previously diagnosed heart disease or diabetes.

"Being discharged from the emergency department is reassuring for patients, but it is critical that they follow up with their doctor to reduce their risks of future heart attacks or premature death," said Dennis T. Ko, M.D., M.Sc., senior author and scientist at the Institute for Clinical Evaluative Sciences, Cardiologist at Sunnybrook Health Sciences Centre in Toronto, Ontario, Canada. "Patients need to advocate for themselves and physicians need to be more diligent about arranging follow-up care."

Researchers analyzed data on 56,767 adults (average age 66, 53 percent men) in Ontario, Canada, who were diagnosed with chest pain in an emergency room between April 2004 and March 2010; had been previously diagnosed with heart disease or diabetes; and didn't require additional hospitalization or die within a month. They tracked data for a median 3.7 years and accounted for differences in key patient characteristics such as age, gender, health status and location.

Among the findings:

  • Only 17 percent of high risk chest pain patients seen in the emergency room were evaluated by cardiologists within a month; 58 percent saw a primary care physicians alone, and 25 percent had no physician follow-up within a month.
  • Patients who followed up with a cardiologist within 30 days were 21 percent less likely to have a heart attack or die within one year, compared with patients who failed to seek additional care within that time.
  • Patients seen by a primary care physician were 7 percent less likely to have a heart attack or die compared to those patients who sought no follow up care.
  • Patients treated by cardiologists received more testing, procedures and medication within 100 days of their ER discharge and had the best health outcomes.
  • Patients seen by their cardiologist were 15 percent less likely to have a heart attack or die within the first year, compared to patients who received follow up care from their primary care physician.

Patients who didn't seek follow-up care within a month received the lowest rate of care and had the worst health outcomes -- demonstrating the need to improve follow-up with high risk chest pain patients after they're discharged from the emergency room, Ko said.

The study notes that there are several reasons patients did not receive additional physician follow-up including: patients believing they didn't need additional care and the lack of a coordinated referral system from the emergency department to physicians who can provide follow-up care.

While there is no cost for seeing a physician at follow-up in the Canadian health care system, the barriers for follow up care could include expense in other countries.

"As physicians, we are often so focused on knowing which drug to prescribe or which test to order that we overlook the fact that many patients fail to get follow-up care to begin with," Ko said. "We need systems of care that better identify these patients who are at increased risk because getting that follow-up can significantly reduce their risks of heart attack or premature death."

Because the study focused on high risk patients, the results may not apply to all who have chest pain. Researchers recommend further study that will also investigate economic factors.

Ashley Madison Creator Noel Biderman Talks Marriage With His Wife Of Nearly 10 Years, Amanda

More than 18 million people in 26 countries frequent AshleyMadison.com, a dating site for married people looking to have affairs.

But while unsatisfied marrieds browse the site looking to hookup, the man behind the site -- creator Noel Biderman -- has been happily married for nearly 10 years. HuffPost Divorce spoke to Noel and his wife, Amanda -- who's featured on the company's new billboard campaign -- in separate phone conversations to discuss the secret to their relationship success, how Amanda reacted when Noel initially told her about his idea for the site, and why Noel believes cheating actually saves marriages.

Amanda, how did you react when Noel first told you about his plan to create an infidelity dating site? Were you supportive? Resistant?

Amanda: Originally, I wanted to make sure it wasn?t an emotional or personal issue...an emotional or physical thing that he was grappling with. Once I understood that it was a sound business [idea], that there was a market he felt was under-serviced, I was totally behind him.

Noel: We weren?t even married at the time, so she easily could have said, ?I think there?s something flawed with this human being.? Instead, she saw the passion in which I wanted to pursue this and she wanted to match that passion. I knew I had a true partner.

Noel, you've said that your website saves marriage and that an affair can serve as a "marriage preservation device." Can you talk a little more about that?

Noel: I definitely believe that. For me, I?m a married man now -- 10 years later, two children later, a really good economic success later, and with an extended family I love, I would be a fool if I said sex was the most important thing in my marriage ?- it?s not. [But] it's important to me, I?m not a priest and I didn?t sign up for a life of celibacy. If I woke up today in some kind of sexless marriage like so many Americans do, I would be genuinely upset by that. I would try to change it with my partner, but if I couldn?t change it, I don?t know if I would just walk out the door. I believe the social science, I?ve seen it firsthand, how children raised in single-parent households have more trouble with drugs and alcohol, have fewer educational opportunities, and get in trouble with the law. I don?t want to do that to my family and I certainly don?t want to do that because everything else I have going for me is great. I like my lifestyle, so why would I give it all up because the number five or six thing on my list -? my sex life -? is not where it should be?

So yes, if my brother came to me and said I can't take it anymore, I'm either leaving or I'm having an affair, I would encourage him to have an affair first.

Amanda, would you stay in a marriage where you knew your husband was being unfaithful for the sake of keeping your family together?

Amanda: I wouldn?t just walk into divorce. But, I mean, I also wouldn?t deal with a relationship where I knew there were interpersonal relations going on without me. We definitely differ on that point. I get that some people are doing this to fulfill a need and to make themselves more fulfilled and happier. I?m not here to judge those people. Personally, that?s not how I live my life. That?s not how I live my life and how I define my relationship and what I would accept moving forward.

Has running a site like AshleyMadison informed how you behave in your own marriage in any way?

Noel: Ironically, I talk about infidelity more than any man on the planet, so hopefully I can navigate a monogamous relationship more successfully than a lot of people. But even then, I don?t know if I can, but I?m going to try.

Monogamy is a man-made notion ?- it?s not in our DNA. It?s not what we?re engineered for. We might think it?s a noble cause and something worth pursuing, but anything against our DNA is something we?re going to struggle with... I?ve seen study after study showing that the longer a couple is together, the more their sex life diminishes. I?ve yet to see any one where it says, ?Twenty years later, those two are still hot and heavy? -? it just doesn?t play out that way. We?re working against something... So it's with that knowledge base that I keep coming back to my own marriage in a different way. While we don?t have an open marriage in a sexual sense, we have a very open marriage in the sense that we can challenge and ask a lot of things of one another because we understand how important our marriage is and how often it can become turbulent.

My wife and I have a very honest relationship with each other -- blunt almost. I think getting things off our chests is the approach we took, knowing that it won?t always go our way but at least it?s out there for the other partner to deal with. That?s kind of suited us. We?re very passionate and when things are great, we ride really high. We also have our moments, but we definitely don?t sleep on the issue ?- and I think that?s the secret to our success today.

Amanda: I think we?re lucky that his role as CEO of Ashley Madison has helped us understand the pitfalls more than anybody else. I also walked into this marriage open-eyed about the realities of relationships because my parents divorced when I was relatively young. I?ve had to deal with the aftermath of that and it wasn?t a nice divorce... I?m 38 years old and I?m still dealing with aftermath of that. I just always wanted to have a relationship where I never ended it in divorce.

Outside of my own experience with divorce, this business has just made us stronger and more communicative. For a lot of people...you meet your spouse, you fall in love, you have this relationship and then kids come and work and life gets stressful and you move so far away from that person that you forget. I think this business has made us remember to always come back to what initially brought us together, so that we can keep the conversation going and not have things build up where we?re looking to move away from each other.

Researchers find immunity protein that ramps up inflammation, and agents that can block it

Mar. 31, 2013 ? Scientists at the University of Pittsburgh School of Medicine have discovered a new biological pathway of innate immunity that ramps up inflammation and then identified agents that can block it, leading to increased survival and improved lung function in animal models of pneumonia.

They reported their findings today in Nature Immunology.

Pneumonia and other infections sometimes provoke an inflammatory response from the body that is more detrimental than the disease-causing bacteria, said senior author Rama Mallampalli, M.D, professor and vice chair for research, Department of Medicine, and director of the Acute Lung Injury Center of Excellence at Pitt.

"In our ongoing studies of pneumonia, we found infecting bacteria activate a previously unknown protein called Fbxo3 to form a complex that degrades another protein called Fbxl2, which is needed to suppress the inflammatory response," said Dr. Mallampalli, who is also chief of the pulmonary division of the VA Pittsburgh Healthcare System. "The result is an exaggerated inflammatory response that can lead to further damage of the lung tissue, multi-organ failure and shock."

The research team, led by Bill B. Chen, Ph.D., associate professor, Division of Pulmonary, Allergy and Critical Care Medicine, conducted experiments in which mice that lacked the ability to make Fbxo3 were infected with a strain of Pseudomonas bacteria, and found that they had better lung mechanics and longer survival than mice that still made the protein.

Research team members Bryan J. McVerry, M.D., and Yingze Zhang, Ph.D., both of the Acute Lung Injury Center of Excellence, found that blood samples from 16 people who had sepsis, a condition of systemic inflammation, revealed higher levels of Fbxo3 and other inflammatory proteins and lower levels of Fbxl2 than samples from seven patients who did not have sepsis or lung infection.

Based on the structure of Fbxo3, the researchers developed a family of small molecules with the aim of inhibiting its activity. Administration of one of them, called BC-1215, led to reduced inflammatory markers and improved lung mechanics in mouse models of pneumonia and sepsis.

"The key is to find ways to help the body temper its inflammatory response so that it's able to kill the infectious agent without causing injury to healthy tissue," Dr. Mallampalli said.

"The F-box protein Fbxo3, and other related proteins, represent ideal targets for treatment of acute lung injury, because it controls the innate immune response, is upstream of important inflammatory signaling pathways, and is more selective than traditional drugs that regulate protein turnover," noted Mark T. Gladwin, M.D., chief of the Division of Pulmonary, Allergy and Critical Care Medicine, Pitt School of Medicine.

The team is beginning to study the effects of BC-125 on other conditions of systemic inflammation, such as colitis and arthritis.

Co-authors of the paper include Tiffany A. Coon, Jennifer R. Glasser, Jing Zhao, Ph.D., Yutong Zhao, M.D, Ph.D., Chunbin Zou, Ph.D, Bryon Ellis, and Frank C. Sciurba, M.D., all of the Acute Lung Injury Center of Excellence, University of Pittsburgh.

Boeing conducts another test flight of 787 jet

(AP) ? Boeing Co. conducted another test flight of its 787 passenger jet over the West Coast.

The company said Monday's flight was designed to test system upgrades and not to check operation of the plane's batteries.

Airlines have 50 of the planes, which Boeing calls the Dreamliner. But they're still grounded after two of them overheated in January. Boeing is trying to assure regulators that after some design changes, the batteries are safe. Boeing expects to make at least one battery-test flight.

On Monday one of the 787s made a 2-hour flight from the Seattle area to Moses Lake, Wash., then returned to Boeing Field. Boeing conducted one test flight of the 787 last week and two in February.

Boeing shares fell 60 cents to close at $85.25. The stock is up 13 percent so far this year.
